Tenth Grade Physical Exam Requirements - Class of 2010

Dear Parent/Guardian:

Policies of the State Board of Education Technical High School System and State Law (C.G.S. section 10-206) require all 10th grade students to have a physical examination and immunization update (current tetanus vaccination, etc.).

The written report of this health assessment must be on file in the school health office prior to the first day of the student's 11th grade year. Proof of this exam must be received by June 13, 2008 to avoid exclusion from school. You may contact me for assistance if you are having difficulty scheduling this exam.

•  Please use the blue State of Connecticut 'Health Assessment Record' form that was mailed to your home and be sure both sides are completed.

•  If your child had a physical exam during or after May 2007, then a copy of that exam will be accepted.

•  If your child enjoys sports/athletic activities, a 'Sport Participation/Medical Examination' form must be completed in addition to the form above.
