FORMS
All athletic participation forms will be issued by the Athletic Coordinator after the sports physical expiration date has been verified.
Interested athletes must go to the CADD Department to obtain these forms.
Once signed off by the Athletic Coordinator, the parents/guardians, and the student, the form needs to be given to the coach.
