Sports Participation Forms
| Please
download and print both forms (medical history questionnaire and
physical form) and bring them to your family doctor. Have the
doctor complete both sheets and return the completed forms to
the School Nurse, Carol Gradowski or the Athletic Director, Ray
Tanguay. As a reminder, you cannot participate in any sport program
until the physical form and the medical history questionnaire
is on file.
Free sports physicals are offered four times a year by our school doctor, Dr. McKay from Torrington. For all freshman and incoming sophomores, the physicals are offered 3 weeks before the beginning of the season. For all returning athletes, the free sports physical in held in June for the upcoming school year. If you have any questions, please contact Carol Gradowski or Ray Tanguay at 860-496-5300.
