Notes from the School Nurse: 

Dear Parents/Guardians of Incoming Students:  Congratulations on your student’s acceptance to Bristol Technical Education Center.  I’d like to take this opportunity to welcome your student and review some important health office information.
             
Requirements for AdmissionA completed health history and emergency contact card and immunization record (enclosed) is required for admission.  If your child is coming from anywhere outside the state of Connecticut, a physical exam completed in the last twelve months and recorded on the enclosed blue form is also required.  These items must be returned to the nursing office in a sealed envelope marked “School Nurse” on the front by: August 15th.  
                                                                                                                                                                    
Physical Exam:  The Connecticut Technical High School System (CTHSS), in accordance with state law, requires that all 9th grade students have a health assessment (physical exam) completed between July 1 following the end of the eighth grade school year and June 21st of  the ninth grade school year.  This physical must be recorded on the enclosed blue form and returned to the School Nurse. It is suggested that Tetanus immunizations be updated every 10 years, especially with students working in the technical areas.  Please be sure to complete the parent portion of the form.  If you have questions about this exam, do not have insurance, or have other circumstances that make it difficult for your child to obtain this exam, please call the School Nurse.  Your sending school will forward a copy of your student’s 9th grade physical and complete health record to the Bristol Technical Education Center.  However, if a physical was not done in the 9th grade then please schedule an exam before school starts.

Health Plan:  If parents do not have a regular health plan, they are strongly urged to purchase accident insurance. Either a 24-hour plan or during school only coverage is available.  Forms will be given out at the beginning of school. Medication:  If your student needs any medication during the school day, the prescriber of the medication must complete a medication authorization form (enclosed). Over-the-counter medication must be delivered to school in its original, unopened container & labeled with the student’s name.  A parent/guardian or other adult must deliver controlled medications (such as Ritalin or Concerta) in the original pharmacy container to the school.  All medications must be given to the School Nurse or an administrator. The only medications that students may carry & self administer are asthma inhalers, diabetes medications and EpiPens.  Students need special permission (noted on the medication order form) to carry these items.  Please contact the School Nurse for more information.  Over the counter medications may be given at school only if the parent/guardian permission is given on the health history and emergency contact card.

Emergency Card:  All parents must complete a health history and emergency contact card each year.  This will be kept on file in the health office, and needs to be kept up to date with current addresses and phone numbers.  These will be given out the first day of school.

Absences:  Parents are required to call the school nurse to report an absence. The school has a voice mail for all calls including evening hours.  The nurse can be reached at (860) 584-8533 ext. 307 and leave a message.

Dear Parent/Guardian of high school student:

Connecticut State Law requires all school children to have periodic physical examinations and to be properly immunized against certain diseases.

All students in 10th grade must submit evidence of a physical examination and adequate immunizations. This includes a blood test for HCT/HGB (a blood test for anemia) and it is also recommended that a Tuberculosis screening be done.

It is suggested that Tetanus immunizations be updated every 10 years, especially with students working in the technical areas. The physical must be dated after the completion of 9th grade and anytime during the 10th grade. It is most important that the school receive the information before the start of school. The student cannot enter Bristol Technical Educational Center without the physical being up to date. Call the school for the necessary forms, if not included with this packet.

Students who must take prescription drugs during school hours must do so under the supervision of the school nurse. Connecticut State Law requires a physician’s written order and parent or guardian’s authorization for the school nurse to administer a prescription or non-prescription medication, this includes medications like Tylenol etc. All necessary forms are available in the health office or main office, if not included with this packet. All medication must be left in the health office. Medication is not allowed to be stored in school lockers or carried on a pupil’s person, except medications for allergies or inhalers, which the parent can sign for self-administration.

All students will receive a copy of an emergency health card which must be kept on file, and be kept up to date with addresses and phone numbers, these will be given out 1st day of school.

It is the responsibility of the parent/guardian to inform the school nurse about any serious or life threatening food or other allergies your child may have. Please see enclosed letter, questionnaire and Health History and return when completed.
