Dear Post Graduate Student:

We are very pleased that you have been accepted to the Bristol Tech Post-Graduate program.  Before you can enter the program, the following medical information must be given to the school nurse.

The Connecticut Technical High School System, as mandated by Connecticut law, requires that all full-time adult students born after December 31, 1956, provide evidence of adequate immunization against Measles and Rubella (German Measles). 

1) Entering students must submit written proof of the required immunizations prior to attending school.
If unable to provide proof of immunizations or you have never received the immunizations, then the immunizations are available at your primary care provider, or walk in clinic.

2) School policy requires that all full time students submit a physical examination prior to acceptance.  The form is included with this packet.

3)  If you need any medication during school hours, the prescriber of the medication must complete a medication authorization form (enclosed). Over-the–counter medication must be delivered to school in its original, unopened container and labeled with your name.  All medications must be given to the nurse. The only medications a student can self administer are asthma inhalers, diabetes medications and EpiPpens.  You will need special permission to carry these.  Please contact the school nurse for more information. Over-the-counter medications, i.e. Tylenol are available at school and forms will be given out the first day of school if you should need this.

5) Health Plan: If you do not have a regular health plan, you are strongly urged to purchase
accident insurance. Either a 24-hour plan or during the school day plan is available.  Forms
will be given out at the beginning of school.
