Re: Tenth Grade Physical Examination Requirement for Class of 2011

Dear Parent or Guardian:

Policies of the State of Connecticut Technical High School System and State Law (C.G.S. section 10-206) require all Grade 10 students to have a mandatory physical examination performed with an immunization update (current tetanus booster, etc.).

If your child is entering the 10th grade in the fall of 2008, then a copy of this physical exam will be accepted if dated on or after May of 2008. Please use the blue 'Health Assessment Record' form that was sent to your home and be sure that both sides are completed.

If your child enjoys sports/athletic activities and would like to participate, please have him/her come by the school health office or physical education director's office to pick up an additional form that must be completed by the healthcare provider.
